ACKNOWLEDGEMENT OF RISK, INJURY AND OBLIGATIONS
I acknowledge that Martial Arts is a dangerous recreational activity and that by participating in it I am exposed to certain risks. I acknowledge and understand that whilst participating in Martial Arts there is a risk of injury, accidents or damage to myself, others and personal property.
RELEASE AND INDEMNITY TO KUNG FU KINDY AND MY CURRENT DAYCARE OR AFTER SCHOOL CARE PROVIDER
I agree to release and indemnity KUNG FU KINDY & TINY KINGDOM as follows:
- I participate in the activity of Martial Arts at my Sole risk and responsibility
- I release, indemnify and hold harmless KUNG FU KINDY & my current Daycare or after school care provider, its employees and agents, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of or arising out of any injury, loss or damage caused to me or my property.
Before filling in this form I have understood it.
WHERE THE PARTICIPANT IS UNDER 1 YEARS OF AGE
WHERE THE PARTICIPANT IS UNDER 18 YEARS OF AGE I, the parent or legal guardian, of the person named in this acknowledgement and release HEREBY ACKNOWLEDGE AND AGREE:
- I have read the whole of this web page and understand it.
- I consent to the person named in this acknowledgement and release, participating un the activity of martial arts.
- I am aware of the risk, dangers, and obligations set out above in this acknowledgement and release.
- I consent to the use of imagery of classes to be used in Social Media posts
IN CONSIDERATION of the person named in this Acknowledgement and Release being accepted to participate in Martial Arts, I agree to release and indemnify KUNG FU KINDY and MY CURRENT DAY CARE OR AFTER SCHOOL CARE PROVIDER in the same manner and to the same effect and extent as if I were the person first named in this Acknowledgement and Release and the person participating in martial Arts.
